  • What if I don't have insurance?
    We accept self pay patients! Our new patient appointments have a one time fee of $150. After that the following visits will be $100 per visit. If we need any additional testing or services those prices will be discussed with you before further services are rendered.
  • What do I need to bring to my first appointment?
    Please bring your photo I.D., insurance card, co-payment, and a list of your current prescription medications with dosage and how often they are taken. If you are a minor, you must have a legal guardian and their identification card as well.
  • How do I sign up for the Patient Portal?
    In order to sign up for the patient portal please make sure we have an up to date email address on file for you. Once we have an email on file you can click here to sign up for the patient portal.
  • What are the benefits of the Patient Portal?
    Through the patient portal, you can message your providers, few lab work results, schedule appointments, and more. You will have 24/7 access to your health.
  • Do you accept walk-in appointments?
    Absolutely! Walk-in appointments are welcome but subject to availability.
  • How do I get my prescriptions refilled?
    You can send medication refill requests via the Patient Portal or by calling our office and requesting a refill. Please note that we do not accept refill request via fax.
  • What is an "Annual Wellness" appointment?
    An annual wellness exam is designed to keep you healthy and provide preventive care. We provide an in-depth review of your health history, physical examination, and personalized health advice. Our goal is to help you stay healthy and identify and address any potential health concerns. Most insurance policies fully cover an Annual Wellness exam once per calendar year.
  • What can I expect from a Pre-Operative appointment?
    A pre-op appointment is what most surgeons require to make sure you have minimal risks for your upcoming surgery. What you can expect varies depending on your surgeons needs and the needs of your primary physician. Typically blood work, imaging, or an EKG can be expected. Please make sure to bring any forms from your surgeon with you to your appointment.
  • How long does a medication prior authorization take?
    Typically 72 hours. However this can vary by different insurance policies. Please give our office a call if you have a medication that needs a prior authorization.
  • What is a medication prior authorization?
    Basically, it is an insurance company needing additional documentation to assure that your medication is medically necessary. This may include sending documentation of previous treatments, lab results and more to the insurance company.
